FRANKLIN, Va. (AP) -- Two companies have been cited for an accident at a paper recycling operation in Virginia that killed a worker from North Carolina.

The Virginian-Pilot (http://bit.ly/13mDrXA ) reports that ST Tissue of Franklin faces state penalties totaling $32,100. CR Meyer & Son of Oshkosh, Wis., faces $70,000 in penalties.

The companies are contesting the citations issued by the Virginia Department of Labor and Industry. The newspaper obtained the citations under the Freedom of Information Act.

Thirty-eight-year-old James Denny III of Fayetteville, N.C., suffered fatal injuries when he was hit by an overhead crane at the Franklin facility last October.

The newspaper says the citations show that state investigators determined that the crane's operator wasn't trained to run the machine. The operator also couldn't see where the crane was going.
