A 50-year-old woman took a fall at home last week, and her daughter called for help.
Responders showed up right away, but the woman refused treatment or hospitalization, according to the Department of Public Safety and Justice.
So the crew put her back in bed and left.
Hours later, the woman was dead.
The apparent breakdown in protocol was the failure of the crew to document its actions by getting a waiver signed by the patient.
If responders did, in fact, break protocol, the case will be forwarded for disciplinary action.
Channel 4 reports the woman had told the EMTs that she was a diabetic and had lupus.
